What Is Achilles Tendinopathy?

Achilles tendinopathy is an overload injury of the Achilles tendon — the large tendon connecting the calf muscles to the heel bone. It causes pain and stiffness at the back of the heel, typically worst first thing in the morning or at the start of exercise. Despite being common, it is frequently mismanaged. Rest alone rarely fixes it — progressive loading is the cornerstone of effective treatment.

What Causes Achilles Tendinopathy?

Achilles tendinopathy is caused by overload — more stress than the tendon can currently absorb. Common triggers include:

Sudden increase in training load

Increasing mileage, intensity or frequency too quickly is the most common cause. The tendon hasn't had time to adapt to the increased demand.

Calf muscle weakness

Weak or fatigued calf muscles transfer more load directly to the tendon. Strengthening the calf is the single most important part of rehabilitation.

Change in footwear or surface

Switching to minimalist shoes, training on harder surfaces or returning after a break can all overload the Achilles suddenly.

Return after rest

Counterintuitively, periods of complete rest followed by a return to activity are a common trigger — the tendon loses capacity during rest and is then overloaded immediately on return.
