What Is a Cervicogenic Headache?

A cervicogenic headache is a headache that originates from the cervical spine — the joints, muscles and nerves of the neck. It is a common but frequently missed diagnosis. Patients often spend years being treated for migraines or tension headaches with limited success, not realising that the neck is the source. The good news: cervicogenic headaches respond very well to physiotherapy when correctly identified.

What Causes Cervicogenic Headaches?

Pain from the upper cervical spine refers into the head through shared nerve pathways. Common causes include:

Upper cervical joint irritation

Stiffness or irritation in the top two or three cervical joints (C1–C3) is the most common cause. These joints have direct nerve connections to the head and face.

Poor posture and desk work

Prolonged forward head posture loads the upper cervical joints and surrounding muscles, sensitising the nerves that refer pain into the head.

Whiplash or neck injury

A previous neck injury — even one that felt minor at the time — can leave residual joint stiffness that triggers recurrent headaches.

Muscle tension and trigger points

Tension in the suboccipital muscles at the base of the skull, the upper trapezius and sternocleidomastoid muscles can all refer pain into the head.

How We Treat Cervicogenic Headaches

Treatment targets the neck directly — not just the headache. This is what distinguishes physiotherapy from most other headache treatments.

Cervical joint assessment and mobilisation

Manual therapy targeting the upper cervical joints is the most evidence-supported treatment for cervicogenic headache — often producing significant reduction in headache frequency within 2–4 sessions.

Soft tissue treatment

Release of muscle tension in the suboccipital, upper trapezius and deep neck muscles — addressing the muscular component of referred head pain.

Deep neck flexor strengthening

Weakness in the deep cervical stabiliser muscles is consistently found in patients with cervicogenic headache. Specific strengthening addresses this underlying cause.

Posture correction

Identifying and correcting the postural habits that load the upper cervical spine — essential for preventing headache recurrence.

Activity and lifestyle guidance

Advice on screen position, pillow height, sleeping position and activity modifications to reduce headache triggers.
